Abstract: Diseases and the related health problems have drastically changed during the last oentury with a rapid increase in the numbers of patients with chronic illness including diabetes mellitus. Diabetes is a slowly progressive quiet disease and hard for the patinets to be aware of its deterioration. However, it contains various risk factors of difficult complications, such as retinopathy, necrosis and ulcer, induced by patients' ill-mannered lifestyle associated with their food-intake, physical activities and so on. Researches have been revealing the diabetes patients' role changing from passive to active participation in their self-care management. They have to confront with repeated crises according to each phase in the course of illness and it is expected that they learn and recognize the importance of acquiring necessary knowledge and skills of active self-care involvement. Also the co-medical staff for a DM patient should work together through a strong support team using the multi-disciplinary supportive technique to communicate and share to facilitate the exchange of imformation and practice. This paper proposes the application of multidisciplinary team approach to supporting the diabetic patients in proceeding effective management of self-care.
